The four Doll Altars are rooms of the Manor of Sleep that are located in the Handmaidens' rooms. One of the four Tattooed Stakes is pierced through the middle of each altar. Each altar also holds a Purity Stone, which are used in the Tattoo Altar's mechanism to raise the cage to the upper level.
Each of the Handmaidens had their very own Doll Altar each located in a certain corner (North, South, East and West). Red dolls were nailed to the walls in each of the rooms with stakes by the Handmaidens as their primary duty when not looking after the current Tattooed Priestess. These dolls represented pain that Worshippers offered at the Kuze Shrine. During the impalement, the Handmaidens would sing the lullaby to appease the pains.
The impalement and the Doll Altars also played an important role in keeping the Rift at bay after the Unleashing. Positioned at the four corners of the old Manor, the Doll Altars marked out the Shrine of Sleep, which was used to prevent the Rift from spreading any further if the Rift Shrine failed to do so.

North[]
The north altar is one of the two that are accessed via the Spirit Tree Courtyard. At the time of the Unleashing it was occupied by Hisame Kuze.
South[]
The south altar can be accessed via an underground passage from the Stained Corridor or by the small door at the bottom of the Grand Staircase. At the time of the Unleashing it was occupied by Amane Kuze.
East[]
The east altar is one of the two accessed via the Spirit Tree Courtyard. At the time of the Unleashing it was occupied by Minamo Kuze.
West[]
Unlike the other three, the west altar is underground and can be accessed via two underground passages, one of which leads from the Wooden Figure Room and the other from the Hall with Tatami. Instead of a door, it has two ladders that lead here. At the time of the Unleashing it was occupied by Shigure Kuze.

- Shrine - "Inside the candle-lit altar, a miniature double doored shrine rests in the place of worship. Inside the miniature shrine, there is a stake made of stone piercing the middle. In the shadow of the stone, at the lower part of the shrine, there is minutely engraved script. "By the balance of light, purify the shadow of the Tattoo Stake. Subdue the priestess in the Chamber of Needles.""
