Ghost Hands are a hazard that first appeared in Fatal Frame IV: Mask of the Lunar Eclipse and have returned in all subsequent titles, including Fatal Frame II: Deep Crimson Butterfly.
Due to the gameplay changing, and forcing the player to manually pick up items, Ghost Hands may emerge to grab the player for minor damage as they are reaching for the item. These hands can be avoided if the player chooses not to pick up the item at that instance. If the player encounters a ghost hand for that item, regardless of whether or not they successfully avoided the hand, a hand will not emerge at the very next instance they try to pick that item up. Consequently, if the player opts not to pick up an item even if a ghost hand does not appear, their next attempt on the same item may have a chance to encounter a hand.
During Fatal Frame IV, hands will only emerge when picking up a Spirit Stone. If a hand successfully grabs the player, the item will disappear entirely. In Fatal Frame: Maiden of Black Water, if the character is wet, the odds of a ghost hand emerging will be increased. Additionally ghost hands now emerge when grabbing any item, including healing consumables and key items, but the item will no longer disappear if a hand grabs the player.
